Activity Night Permission Slip

I give my son/daughter ________________________________________permission to

attend the activity night on ______________________________  from 7 to 9:30.

I understand that my child should not have his/her cell phone on during these hours.  I understand that if the cell phone is seen it will be taken and it will necessitate me picking it up at the end of the night. 

I understand that I must provide transportation promptly at the specified time.  I also understand that chaperones will be available up to 15 minutes after the event is over.  Students remaining after that time will be subject to administrative action.

Remember…Only students enrolled in Macopin School or 7^th or 8^th grade in Queen of Peace and living in town are permitted to attend.
